Care Gap Enhancement Program
One of the assumptions that guide the contemporary value-based healthcare landscape is that as you increase provider investment in performance measurement and compliant documentation and reporting there is a corresponding increase in healthcare quality and efficiency. The Guardian Health Services Care Gap Enhancement Program, driven by the Guardian IT platform, is designed in direct response to this now widely imbedded assumption regarding provider performance measurement. The central goal of the Program is to provide client healthcare organizations and providers with a timely and efficient patient specific process for:
- Identifying specific gaps in performance measurement.
- Organizing and prioritize scheduling of patients to close identified performance measure gaps.
- Engaging in fully compliant documentation and reporting of closing on-going identified performance measurement gaps.
- Enhancing revenue through complete, accurate, and timely quality performance measurement capture and reporting (e.g., STAR performance reporting; MIPS quality performance score).
Guardian Health Services clinical staff:
- Assist contracted clients in identifying, organizing, prioritizing, and sequencing patient specific performance measurement in direct compliance with specific performance measurement sets.
- Educate practice staff on the value of the using the Guardian platform and the GHS Care Gap Enhancement Program in the process of satisfying their performance reporting requirements.
- Continually reinforce with practice staff the importance of outcome performance measurement and the use of performance data for continuous quality improvement within the practice setting.
- Assist the client in building a culture that is responsive to on-going and timely performance measurement documentation and reporting.
Guardian Health Services provides opportunities for the staff of clients to attend weekly GHS Care Gap Enhancement Training Webinars (via remote internet access or face-to-face in the GHS office) on technical details of performance measurement sets and the specific steps for interfacing with the Guardian HIE platform in identifying, closing, and documenting specific performance measures. In addition, performance outcome reports are systematically generated for review by clients.
