Guardian Health Services
The MISSION of GHS is to positively impact the delivery and outcomes of healthcare through client contract-based implementation of innovative programs interfaced with a robust HIE IT platform that ensures timely, secure, effective, and meaningful clinical integration, care coordination, and care management.

The operational goals of GHS are consistent with the contemporary movement toward reducing fragmentation and escalating costs within the health care system, and the evolving paradigm shift from fee-for-service volume based medical care to fee-for-value based-care built on a foundation of action oriented accountability. GHS is dedicated to working with healthcare organizations to:
- Reduce unnecessary use of health services (hospitalizations, emergency room visits) due to modifiable causes (e.g., change in risk behaviors, medication compliance, reduction in risks for falls, medication management to avoid poly-pharmacy).
- Facilitate collaborative relationships between patients, primary care, specialist, and ancillary providers, and their care managers, through a partnership or team based approach to care coordination and care management, including effective integration and coordination of available health, mental health, and social services personnel and resources.
- Establish and continually reinforce collaborative communication among between primary care physicians, specialty physicians, hospitalists, ancillary providers, and care managers, regarding where care is delivered (emergency department, urgent care facility, hospital, physician's office, patient's home or another setting).
- Reduce unnecessary disparities in the delivery of healthcare services provided to high risk patients through the provision of targeted chronic care management and proactive coordination of services and assistance in navigating the health, mental, and social service system.
- Improve the health and the quality of life of patients through interventions that identify, inform, and educate patients, family members, providers and other partners in healthcare.
- Facilitate the development of effective self-management skills and enhanced empowerment and engagement among patients presenting with complex chronic morbidities, and/or high risk lifestyle behaviors.
- Improve appropriate medication adherence, self-monitoring, and appropriate lifestyle changes.
- Improve clinical outcomes through compliance with nationally recognized and accepted evidenced-based clinical practice guidelines for prevention, management of high-risk health behaviors, and treatment of chronic conditions.
- Integrate clinical care and services with non-clinical community interventions (e.g., personal care services; meals on wheels).
- Systematically measure, track, and improve outcome performance and outcome improvements utilizing widely recognized reliable and valid performance measures and methodologies (e.g., HEDIS measures; STAR Measures).
